This is a form I developed years ago when I first joined the L&D industry as a Training Coordinator, brand new to instructional design. I was assigned to a project with a client and needed to interview each business unit to perform needs analysis on their training needs. Pulling together what I learned from my instructional design certification course and some additional research, I created this template and off I go!
All these years later, every single time I’m about to embark on a new training project, I find myself pulling this back up and using it as a guide for my intake interviews.
